PHP Foundation Helps Goshen
For the fourth year in a row, the PHP Foundation is helping Maple City Health Care Center to provide health care for people in Goshen.
This year, the PHP Foundation awarded a grant of $110,000 to help Maple City Health Care Center care for people with diabetes.
The grant will support a wide range of services including a chronic disease care coordinator. The care coordinator is a bilingual nurse. The nurse will provide both one-on-one coaching as well as Healthy Living Groups.
About a third of Maple City’s patients are unable to obtain health insurance and about half of the patients speak Spanish.
The PHP Foundation is a program of Physicians Health Plan, a non-profit health insurance company in Fort Wayne, Indiana. Each year, the PHP Foundation helps non-profit charities in northern Indiana. The grants totaled $10 million in the past 15 years
Maple City Health Care Center provides affordable, high quality, integrated health care. Maple City’s services include health care for sick and injured people, immunizations, obstetrical care, behavioral health care, dental care, eye care, chronic disease management, healthy living groups, nutrition counseling, and substance use disorder treatment.
“The PHP Foundation grant helps us improve the quality of our services,” explained said Dr. James Nelson Gingerich, Maple City’s Guardian of Vision. “During the next several months, we will connect with many of our patients by phone and video. The PHP grant is even more important now because people with diabetes are especially vulnerable to the effects of COVID-19.”
